NEW ROCHELLE, NY — The City School District of New Rochelle Health Services Department issued a reminder this week to families of students in the District, encouraging them to visit the department’s website to obtain health-related forms for school and athletics, review immunization and health examination requirements and access health office contact information.
To assist the department in providing the best care for your children during the school year, please return all completed forms to your child’s school nurse. If you have not already, contact your child’s medical provider for the following:
- If your child is due for vaccines, schedule an appointment as soon as possible. Notify the school nurse of your appointment to prevent unnecessary exclusion from school.
- If your child is a new student or will be entering kindergarten or grades 1, 3, 5, 7, 9 or 11, have your doctor fill out a health examination form.
- If your child will require medication during the school day, please obtain updated orders from your doctor before school starts. To ensure proper delivery and receipt, we request that parents/guardians bring all medications/supplies directly to the nurse on the first day of school.
